Bilevel devices can offer very different therapy modes. Labels such as S, ST, auto bilevel, AVAPS or iVAPS describe different control strategies; they should not be treated as a ladder where the device with the most modes is automatically better.
S mode
Spontaneous bilevel therapy generally follows the user's own breathing efforts while providing separate inspiratory and expiratory pressures.
ST mode
Spontaneous/timed modes add a backup timing function. The need for a backup rate is a clinical decision, not a shopping preference.
Auto bilevel
Auto-adjusting bilevel devices can modify therapy within configured parameters depending on the device algorithm and prescribed setup.
Volume-assured modes
Technologies such as Philips AVAPS or ResMed iVAPS are manufacturer-specific strategies intended to support ventilation targets. They require appropriate clinical selection and setup and are not equivalent merely because both are “volume assured.”
